Clinical Pharmacist Part Time Rotating Position Summary:

The clinical pharmacist will be a unit-based pharmacist that will verify the orders and provide clinical services for the area(s) for which they are assigned. They will be a drug information resource for the nurses, physicians, patients, and families.

Responsibilities

This individual’s responsibilities include (but are not limited to):

  • -Order verification and performing medication profile reviews that should include but not limited to: checking for duplications, drug- drug and drug-food interactions, allergies, drug appropriateness, dosing, and frequency.
  • -Check High Risk IVs, renal dose adjusting medications, IV to PO conversions, initiates/modifies/ discontinues therapy, and mediates black box warning interventions.
  • -Provide for collaborative practice management as per facility-approved protocols for anticoagulation, parenteral nutrition, vancomycin and aminoglycoside consults, etc.
  • -Assist with medication reconciliation and clarification of unsafe and unclear drug therapy orders.

THE CLINICAL PHARMACIST PART TIME ROTATING WILL POSSESS THE FOLLOWING EDUCATION, LICENSE/CERTIFICATIONS, AND EXPERIENCE

Education: Pharm. D. or BS in pharmacy

Minimum Experience: 3 years or residency training or equivalent experience

Certifications/Registrations/Licenses:

Required: Pharmacy License; BLS within 30 days of hire;

Preferred: Board Certification in Specialty area, CGP, ACLS, etc.
