Occupational Therapist Per Diem Rotating Position Summary

Position administers specified evaluations, conducts general activity programs and assists in implementing specific treatment programs under the

guidance of a Occupational Therapist. Functions as an interdisciplinary team member under the direction of an Occupational Therapist. Evaluates

activities of daily living status, as well as assists the OTR in the implementation of a formulated treatment plan, enabling patients to reach optimum

functional performance. Provides quality occupational therapy services according to the principles and practices of occupational therapy.

SPECIAL SKILLS: Excellent customer service skills

THE OCCUPATIONAL THERAPIST PER DIEM ROTATING CANDIDATE WILL POSSESS THE FOLLOWING EDUCATION, LICENSE/CERTIFICATIONS, AND EXPERIENCE.

MINIMUM EDUCATION: Graduate of an Accredited School of Occupational Therapy Assistant Program.

PREFERRED EXPERIENCE: One year.

REQUIRED CERTIFICATIONS/LICENSURE: Certified as an Occupational Therapy Assistant by the American Occupational Therapy Certification

Board. Current license in the State of Arizona. CPR.
