JOB SUMMARY

The Care Coordinator III will be responsible for providing lead support to market Supervisors and Managers. Provides cross coverage for hospital based Care Coordinators. Provides account level and reporting analysis for supervisors and hospitals. The Coordinator will also be responsible for participating in marketing the Care Continuity program with the assistance of the business development team to the affiliated provider network. The Coordinator lead will visit physician offices/outpatient providers, introduce the offices to the Care Continuity program, provide the offices with logins and passwords, and collect the appropriate signed security documents from the offices.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Assists market leadership with the review and documentation of employee work effort in terms of quality, volume and provide feedback to staff on audit outcomes.
  • Assists market leadership with the compilation and analysis of various monthly reports.
  • Act as player/coach to offer hands on support across multiple facilities/markets.
  • Provide cross coverage for hospital based Care Coordinators
  • Conduct monthly audits and compile reporting for supervisors/managers
  • Participate in marketing with the assistance of the business development team including physician and clinic visits

  • Ability to work independently
  • Detail oriented, with strengths in dealing with multiple facilities, Supervisors and hospital staff.
  • Ability to interact well with ER staff so that navigator is positioned/seen as another member of clinical care team by the patient
  • Ability to develop good relationship with hospital staff so that navigator can work as part of case management team and point of contact for the outpatient environment
  • Ability to develop trusted relationship with outpatient physicians in order to improve appointment scheduling and communication between post-acute care providers and the hospital.
  • Ability to communicate extremely well via phone and email to both staff and management
  • Ability to travel to client facilities around the US staying overnight, possibly for several nights in a row
EDUCATION / EXPERIENCE

Include minimum education, technical training, and/or experience preferred to perform the job.

  • Healthcare provider operations in at least one key setting (e.g., hospital, physician�s office, long-term care, etc.)
  • LVN or equivalent is preferred

  • Includes ability to walk through hospital-based departments across broad campus settings, including Emergency Department environments
  • Long distance travel demands (e.g., drive for extended periods, fly on airplanes, walk through airport with luggage, etc.)
