Summary
The Heart and Vascular Institute of Texas is seeking a Coumadin Nurse.
The Coumadin Nurse is responsible for patient triage, patient education, and assisting physicians in the assessment of patient needs and treatment plan(s).
Maintain chart documentation to ensure all information about patient is completed and documented correctly. Occasional local travel to support other clinics required.
Should be able to properly provide dietary education to patients taking Coumadin and explain the importance of Coumadin and Vitamin K management.
Should know how to identify and education the patient on how to identify the signs and symptoms of adverse reactions to Coumadin, what to report to their provider and when to report their reactions/ concerns.
Should know what medications interact with Coumadin and how to educate the patient on proper management of these medications.
Should be knowledgeable of various diagnoses that a patient may require INR management. For example: Atrial Fibrillation, DVT, Mechanical Valve and Pulmonary Embolism are a few examples of diagnoses and they each may have a different INR target. Not all INR targets are the same for each patient.
The Coumadin nurse should know how to educate a brand new patient starting out on Coumadin for the first time.
Qualifications:
Education
High School Diploma or equivalent and completion of an accredited nursing program.
Experience
2 years of clinical in a physician office.
Previous coumadin experience required.
Certifications
Current State RN license and CPR certification.
