Description:

POSITION PURPOSE:  Responsible for timely, accurate and complete scanning of patient’s medical records.  Ensures maintenance of daily workflow, ensuring that records are scanned in a timely, accurate and efficient manner to avoid delay in further processing of records.  Ensures that all patient records and loose documents are scanned into the Meditech ELR with the highest level of quality possible.  Indexes all documents to the appropriate visit and ensures that each document is assigned to the appropriate form ID.  .   .

PRINCIPAL DUTIES AND RESPONSIBILITIES:

1.Medical Record Assembly: Retrieves all records of discharge patients from the floors and locates missing records. Checks daily discharges log to ensure receipt of all records and follows up on records not received.

Assembles records according to established procedures.

Incorporates loose paper work and thinned records into records of discharged patients.

Chart Prep for Scanning: Prepares paperwork for scanning according to established procedures (e.g. remove staples, place already scanned documents on top behind the facesheet ect.)

Review forms/documents for a visit to ensure that they belong to the correct patient.

Secures loose undersized documents appropriately.

Places appropriate documents in date/time order and appropriate forms in page order.

2.Chart Scanning: Copies all damaged documents to ensure that they go through the scanning process without difficulty.

Identifies and removes archived/cold fed documents from each visit as 

                       appropriate.

                       Retrieves records from scanning basket and performs scanning routine according 

                        to procedure and within required time frame.

                      Identifies pages that do not scan well and flags for scanning on flatbed scanner.

                      Correctly inserts pages/documents when and where appropriate.

                      Accounts for each document ensure that all documents were scanned.

                      Performs the AFI routine according to established procedures and in a timely 

                       manner.

                      Understands documentation requirements and utilizes that knowledge to assign 

                       appropriate form ID barcodes.

                      Ensures that all scanned documents are positioned correctly.

                     Verifies that bar-coded documents are correctly identified.

                     Choose the appropriate visit and ensures that documents are scanned to the 

                      appropriate visit.

                      Ensures that each document is scanned to the correct patient.

                      Performs the rescan procedure as appropriate.

                      Prints scanning and archiving report and accounts for every record that has been 

                      scanned.

                     Completes cover sheet, bundles batch together and places completed batches in t

                      the appropriate box/area.      

                      Cleans Scanner on a daily basis and weekly basis according to established 

                       procedures.        

3. Other:  Empties buckets from FUH upon arrival and distributes contents accordingly.

Coordinates all retrieval requests to ensure that all requests are responded to in a 

               timelymanner.Takes down appropriate information to ensure that all requests are 

               responded to in a timely  manner.Ensures that medical records are labeled correctly 

                (transmittal slip filled out properly) and sent to ````````````the correct location.

4.Downtime Procedures: Correctly performs downtime procedures for looking up medical record numbers  when Meditech is down. Prints appropriate reports on preparation for possible downtime.




Qualifications:

KNOWLEDGE, EXPERIENCE AND OTHER JOB REQUIREMENTS:

High School education or equivalent.

Basic Computer skills required

Able to read, write and speak English

No formal experience required.  Ability to act independently.

1.PHYSICAL REQUIREMENTS:

Physical health sufficient to meet the ergonomic standards and demands of the position and its location.

Ability to perform all physical activities associated with filing and retrieval of medical records (lifting, carrying, bending and stretching).
