Up to $15,000 Sign-On bonus based on experience

Position Summary

The Staff Physical Therapist is responsible for providing therapy services, upon referral from an appropriate licensed practitioner, to patients including assessment, treatment planning, and implementation related documentation and communication. Evaluates and treats patients in order to produce effective outcomes. Communicates and assists in resolving problems. Demonstrates effective resource utilization that is critical to operational stability within the organization. Participates in performance improvement and program growth.

1. Reports for scheduled shift on time and prepared to assume responsibilities.               

  • Reports to work in proper uniform or attire.     
  • Leaves for meal break on time and returns on time.
  • Varies work schedule to meet the needs of the department.
  • Ensures all overtime has been approved by the department head prior to working.
  • Schedules all time off with supervisors approval. 
  • Adheres to Kronos policy on properly punching in/out for each shift and accurately recording education, interdepartmental, and student times
  • Strictly adheres to hospital attendance/tardiness policies 100% of the time.
  • Performs all duties as assigned.   
  • Wears I.D. badge above the waist with name clearly visible at all times on hospital property.

 

2. Attends and participates in meetings and is responsible for all information communicated at meetings.

  • Participates as an active team member at all staff meetings.
  • Reads all communications and acts upon in a positive and informative manner.
  • Attends all hospital mandatory in-services on a timely basis as scheduled.
  • Is an active participant in assigned Performance Improvement Group meetings.

         

3. Adheres to Corporate, Regional, hospital and departmental policy and procedure 100% of the time.

  • Reads and adheres to the company's Standards of Conduct. 
  • Attends Ethics and Risk Management training annually.
  • Completes annual competencies and mandatories in a timely manner.
  • Understands that compliance with Standards of Conduct and Hospital Policy and Procedures is a condition of employment, and violation of any principles or practices of the program will result in disciplinary action up to and including termination. 
  • Maintains confidentiality regarding patient/employee/ facility/corporate information 100% of the time. 
  • Accepts change as an opportunity for growth, learning and development; adapts to changing procedures and goals in a cooperative and positive manner.

 

4. Assists all patients, family members, physicians, visitors and employees in a professional and courteous manner.

  • Demonstrates empathy and sensitivity to all patients, visitors, physicians, and fellow employees.
  • Assists in maintaining an atmosphere of cooperation with other departments and allied professionals.
  • Demonstrates respect and cooperation in all staff relationships, and a genuine willingness to prevent or resolve inter-personal conflicts.
  • Demonstrates the ability to participate in and/or implement team decisions.
  • Actively participates in and encourages actions that promote good public relations with patients' families, visitors and the community.
  • Works together in a spirit of teamwork.
  • Shares information on a "need to know" basis.

      

5. Adheres to the following environmental care standards.

  • Demonstrates adequate knowledge of Safety Policies and Procedures, the use of MSDS sheets, Hazardous Material procedures and Safety Codes.
  • Demonstrates adequate knowledge of Security issues, i.e. personal protection, protection of personal and hospital property.

 

6. Performs and fulfills all HICS (hospital incident command system) responsibilities, assignments, tasks, and roles as directed and needed by the facility in times of emergency and/or disaster. Responsible for knowing the Hospital’s emergency codes, including the purpose and appropriate action for each, and adheres to the Hospital polices and procedures for each emergency code. 

 

7. Demonstrates the ability to adapt to varied age-specific patient populations

8. Complies with all policies & procedures for this position that pertain to HIPAA.

  • As a part of the requirements for this position, the employee has access to the entire medical record for the purpose of providing direct patient care throughout the continuum of care
  • Limits the protected health information (PHI) disclosed or requested to the amount reasonable necessary to achieve the purpose of the request within HIPAA guidelines
  • Makes disclosures for the purpose of appropriate patient care and of meeting local and state requirements for reporting
  • Criteria established to limit the PHI disclosures/requests to that which is reasonable necessary includes:  communicating only the PHI that is required by law, upon request & for the correct/intended purpose of providing patient care & notification to local/state authorities &/or regulatory agencies.

Qualifications

EDUCATION

  • Minimum: Bachelor of Science Degree 
  • Preferred:  Master’s Degree in Physical Therapy
     

EXPERIENCE

  • Experience desired, but not required
     

REQUIRED CERTIFICATION/LICENSURE/REGISTRATION

  • Florida license as a Physical Therapist from the Agency of Health Care Administration’s Division of Medical Quality Assurance.
  • Current AHA BLS CPR card
