EDUCATION:

Minimum:        Associates Degree - Graduate of an AMA respiratory therapist program.

Preferred:        Bachelor's Degree

EXPERIENCE

2 -3 years clinical experience as a Registered Respiratory Therapist

REQUIRED CERTIFICATION/LICENSURE/REGISTRATION:

Current Florida License as a Respiratory Therapist, NBRC RRT credential 

American Heart Association ACLS and BLS certifications

CPFT or RPFT preferred

OTHER QUALIFICATIONS: 

COPD Educator course, RT Case management course preferred

Position Summary:

The Pulmonary Disease Navigator is a Registered Respiratory Therapist (RRT), who will be responsible for working directly with patients who are diagnosed with chronic pulmonary diseases. These diseases include chronic obstructive pulmonary disease (COPD), emphysema, pneumonia, asthma, and pulmonary fibrosis. The Pulmonary Disease Navigator will act as a liaison to patients, families, and providers to ensure proper length of stay, adequate education, and safe discharge. They will ensure that quality metrics specific to the disease process’ are met and promote compliance with management of chronic pulmonary diseases throughout the continuum of care. Lastly, the navigator will provide referrals for follow-up testing and/or to other disciplines and specialties when appropriate.

The Pulmonary Disease Navigator will successfully complete a generic and department-specific competency assessment. The Navigator will have experience with pulmonary function testing and the GOLD Standards used in diagnosing COPD and six-minute walk.

Experience in Pulmonary Rehabilitation, Cario- Pulmonary Stress testing and will develop and participate in Community outreach programs. Responsibilities will include ensuring that the patient has received skilled and competent delivery of Respiratory Care Services, as ordered by a physician in accordance with established protocols set by the facility or other health care provider in the treatment, management, control, diagnostic evaluation and care of patients with deficiencies and abnormalities associated with the Cardiopulmonary system. These services may include but are not limited to: delivery of medical gases, pulse oximetry for oxygen titration, administration and monitoring of all respiratory and therapeutic modalities which include aerosolized medications, metered dose inhalers (MDI), sputum inductions, and arterial blood gas sampling and analyses. Current practice in area of responsibility; required computer/information systems and equipment used in performing assigned duties; application of safety and infection control policies and procedural customer feedback and impact on core processes; verbal and written communication skills that are sensitive to diversities among age groups, cultures, and education levels facilitating teaching. Supports staff orientation, in-services, and competency needs regarding pulmonary diseases. Coordinate follow-up with patients and hold monthly COPD support group meetings to ensure we are meeting the needs of our patients both in and out of the hospital.
