General Functions:

Under the supervision and guidance of a RN, demonstrates knowledge of care and differentiates abnormal conditions; accurately documents vital signs, intake and output and reports deviations from normal to the nurse in a timely manner
•Under the supervision and guidance of a RN, performs all responsibilities/duties required under the defined scope of service to assure the unique nature of the client is addressed to include but not limited to the age of the client served
•Institutes and initiates a plan of care for patient based on the initial assessment within the shift, documents; implements and continuously evaluates interventions; reassesses patient needs on an on-going basis as required per unit or procedure
•Performs all relevant duties to include but not limited to preparation of units and duties associated with patient room and beds for receiving patient admissions, returning or outgoing surgical patients, transfers and discharges without reminders
•Demonstrates ability to care for patients with an ostomy, diabetic patient, patient with impaired skin integrity, bariatric patient, and perform peri care
•Demonstrates ability to care for geriatric patients with specific needs related to acute illness that may have an impact on their mental status (increased confusion, delirium)
•Demonstrates ability to maintain infant security and address nutritional needs based on developmental age
•Initiates Fall Protocol for all patients; assists with placement of arm band, appropriate colored slippers and appropriate signage
•Maintains work area clutter free, equipment in working condition required by department standards; Keeps utility and equipment rooms and work areas clean
•Demonstrates knowledge of the use of medical equipment (i.e.. Sequential compression device, bedside commode, bladder scanner, mobility assistive devices); reports problem with equipment to charge nurse via change of command
•Recognizes changing patient condition and modifies plan by using resources, chain of command and Rapid Response Team (RRT) promptly and appropriately.
•Assimilates and prioritizes information sources to take immediate and decisive patient-focused action by routinely performing patient rounds and addressing concerns timely
•Assists patient with activities of daily living and provides basic care to include but not limited to patient hygiene, preparation of meals or assistance to feed as needed, distribution of water and ice
•Performs routine treatment procedures including but not limited to application of appliances for heat/cold sitz baths or postmortem care per policy as directed by nurse
•Maintains a safe environment and responds with confidence and adapts to rapid changing patient conditions and emergent situations
•Operates equipment and performs all procedures safely, including use of proper body mechanics and protective personal equipment
•Appropriately handles supplies on unit, correctly and consistently removes and charges supplies appropriately
•Adheres to privacy and maintains confidentiality of all hospital and patient information at all times
•Performs phlebotomy (venipuncture, fingerstick, heel stick and bleeding time) according to established procedure and protocol of hospital by using correct technique.
•Performs phlebotomy procedures with minimal discomfort and performs bleeding times according to procedure
•Determines priority of STAT draws and ensures timely transport on a priority basis for serious cases; severe trauma and/or disaster patients are drawn immediately, STATs within 10-15 minutes
•Performs electrocardiograms in assigned areas timely and communicates findings to appropriate personnel timely to include but not limited to filing EKG tracings at end of shift
•Assures specimens and EKG results are labeled, transported, accounted for and performed per hospital protocol/procedures using patient identifier process to include but not limited to correct date/time, phlebotomist initials
•Assists coworkers with phlebotomist procedures as needed, communicates patients not drawn and/or concerns to Nurse/Charge Nurse
•Appropriately documents all patient specific information t and escalates identified problem for care provided
•Coordinates and collaborates with multidisciplinary team members in the planning and management of patient care to meet patient needs and maintains high productivity
•Responds to the unique needs of patients and families coping with unanticipated treatment, quality of life, and end-of-life decisions

•Facilitates patient movement within the department and through the facility, proactively managing patient admission, discharges and transfers including the ability to prioritize to maximize efficiency and recognize acuity levels
•Effectively manages patient hand-offs at shift change and between department and facilities; utilizes documentation tools for hand-offs as required
•Demonstrates ability to transport patients safely as needed
•Abides by regulatory agencies standards and provides every patient the defined standard of care
•Utilizes resources efficiently and effectively by demonstrating independence and resourcefulness
•Adheres to Tenet Standard of Conduct as well as hospital policies and procedures
•Attends safety training programs and demonstrates adherence to proper hospital exposure procedures
•Is self-driven and actively pursues personal and professional development including a constructive approach with colleagues
•Addresses patient population-specific including an inclusive culture
•Actively participates in patient satisfaction initiatives, to include hourly rounding to address concerns; is able to identify patient or family concerns and escalates via chain of command in a timely manner
•Demonstrates willingness to identify and/or assume activities relative to the developmental needs of the unit and department
•Collects and transports specimens to correct department in appropriate and timely manner
•Performs all responsibilities/duties required by the specific unit as defined in the scope of service and demonstrates effective unit specific skills
•Maintains required competencies, certifications, online education and assures orientation to floating unit is completed by deadline date
•Verbalizes and demonstrates ability to escalate unsafe conditions throughout the hospital
•Respects privacy and confidentiality of sensitive information and situations
•Balances team and individual responsibilities and is objective and open to others' views
•Demonstrates dependability/flexibility and assumes other duties and responsibilities as assigned

Full Time Days

Summary
Assists nursing service personnel with a variety of patient care activities and related services necessary in caring for the personal needs and comfort of patients. Assists with personal hygiene, takes vital signs, and changes bed linens. May serve and collect food trays, provide for between-meal nourishment, and clean, sterilize, store, and prepare other supplies. Has knowledge of commonly used concepts, practices, and procedures within a particular field.

Education:

Required - High School Diploma or GED

Experience:

Required - None
Preferred - 6-month acute care and/or previous clinical experience

Certifications:

REQUIRED LICENSES/CERTIFICATIONS:
Completion of a nursing assistant program
Current Healthcare Provider Basic Life Support (American Red Cross or American Heart Association) if American Red Cross, must obtain American Heart Association within 60-days of hire
Crisis Prevention Intervention (CPI) Certification within 90-days of hire/transfer
