Description

At Hi-Desert Medical Center (HDMC) we've been caring for our community for over 40 years. Our Continuing Care Center (CCC), located on the campus of HDMC, provides specialized care to residents close to their own communities, with spectacular panoramic views of desert landscapes for both residents and their guests

The Continuing Care Center has 120 beds and offers three distinct levels of service in semi-private rooms:

  • Subacute care: This is the highest level of care available in our long-term care setting. The CCC provides 25 beds dedicated to residents with ventilator and tracheostomy requirements. Clinical staff is specially trained to meet the unique needs of these patients. The CCC is one of the few facilities in Southern California providing this level of care.
  • Skilled nursing care: We provide long-term skilled nursing care to residents who require assistance with personal and medical needs. Care is provided to our patients who require rehabilitative therapies, wound care or recovery from accidents or invasive procedures.
  • Long-Term Care: We provide long-term care to residents who require assistance with personal and medical needs.

Position Summary

Assists with quality patient care by being sensitive to the needs, interest, and abilities of residents.

Shift: Full- Time Days Mon,Tue,Wed, Sat and Sun

Hours: 8am-4:30pm

Responsibilities:

Primary Duties 

  • Works with Activities Coordinator and/or Continuing Care Center Administrator to provide appropriate activities to a diverse group of residents at the CCC.
  • Coordinates and cooperates with all staff members and facility services.
  • Compiles and maintains current records, plans, reports and evaluations of activities programs and resident participation/functioning as assigned.
  • Participates in orientation and in-service education programs as assigned. 
  • Assists in creating monthly calendar for scheduled resident activities.
  • Instructs participants on how to use materials or musical instruments or other relevant activity materials. 
  • Assists with coordination and facilitation of monthly Resident Council meeting.
  • Makes room visits to residents unable to attend activities and provides in-room activities.
  • Assist in maintaining calendars and seasonal/holiday appropriate decorations throughout the Continuing Care Center.
  • Works with other facility staff to improve attendance at activities by coordinating transportation to and from events with clinical staff.   
  • Employee is self-directed with tasks assigned and assumes leadership of the activity department when the Activities Coordinator is absent.
  • Maintains all documentation assigned by the department or as required by state and/or federal regulations or by Joint Commission.
  • Performs other related duties as assigned or requested.

Qualifications

Physical demands: 

  • Assists with lifting and moving patients
  • Considerable reaching, stooping, bending, kneeling and crouching 
  • Lifts supplies/equipment
  • Manual dexterity and mobility 
  • Moderate physical effort (lift/carry up to 25 lbs.)
  • Occasional prolonged standing/walking

Working Conditions: 

  • Contact with patients under wide variety of circumstances
  • May be exposed to infections and contagious diseases
  • Work alternating weekends and holidays

Minimum Education

  • High School Diploma or GED

Certification/Registration

  • Activities Designee Certification required within 6 months of hire.

Required Experience

  • Minimum 1-year recent experience working as activities assistant and/or coordinator in a Long Term Care facility
  • Experience in Microsoft office programs

Pay Range: $24.00 - $24.49 hourly **Individual wages are determined based upon a number of factors including, but not limited to, an individual’s qualifications and experience
