LVN SNF Extended Care Full Time Nights

Primary Duties
- Maintains current Professional License (LVN) and appropriate CPR Certification and provides facility with a copy of these documents in a timely manner to ensure practice is covered by active License and Certification at all times. Keeps up-to-date educationally in areas of Specialty
- Completes initial and annual competencies and annual Health and Safety requirements timely. Orients new LVN’s as assigned. Assists in appropriately orienting registry and float staff as assigned by Charge RN. Is helpful, positive and supportive when working with registry and float staff.
- Knowledge of Residents Bill of Rights and Elder Abuse reporting regulations. Observes Residents Bill of Rights and reports incidents of elder abuse, if indicated. Carries out duties in a manner sensitive to and appropriate to age (primarily adults and older adults), culture, religion, gender, and sexual orientation of the residents.
- Control the environment to prevent hazards and cross infection by observing HDMHD policies and procedures. Attempts with guidance from the Charge RN, alternatives to physical and chemical restraints are attempted before requesting use of physical and chemical restraints. Is familiar with SDS information sources on the Unit.
- Communicates with Charge RN, other team leaders and CNA staff, residents, family members and visitors, to ensure a professional two-way flow of information regarding resident care and other issues. Assesses residents daily and on an as needed basis, and communicate concerns to the Charge RN using sound nursing judgment and decision making skills. Communicates regularly with the Clinical Director regarding resident care and other issues not resolved by discussion with the Charge RN.
- Helps ensure department operates in a financially sound manner by using supplies and resources appropriately.
- Carries out on a daily basis, and/or as needed:
- Administers and documents ordered medications and treatment according to standards of practice for Skilled Nursing Facilities
- Maintains contents of medication carts and orders individual and floor stock supplies as needed
- Follow regulations regarding locking of medication carts and labeling/dating of medication bottles Ensures body fluids for ordered test are obtained timely
- Follow correct protocol with regard for administration and documentation of controlled substances
- Completes weekly summary on residents and updates care plan as needed
- Only LVN with Phlebotomy/IV certification start IV’s and obtain blood samples
- Completes assigned admission documentation and assessments
- Communicates need for equipment or supplies to Charge RN or Unit Secretary
- Participates in PI and PIT team as requested
- Complies and ensure others comply with all resident privacy and HIPPA requirements
- Participates in monthly recapitulation process as requested
- Ensures CNA’s complete assigned resident care tasks
- Provides resident and family education and training as assigned
- Ensure DPAC/Advance Directive wishes carried out in accordance with CCC policy and procedure
- Completes Incident and Opportunity to Improve forms
- Completes assigned discharge and other documentation
- Participates in resident Care Plan meetings and ensures care plans are appropriate and up to date.
- Ensures if injured reports and obtains treatment as needed per HDMHD policy
Qualifications
- Current Licensed Vocational Nurse (LVN)
- BLS Certification
- IV/Phlebotomy certification is preferred

Position Summary
Coordinates resident nursing care on the SNF/CDP Unit.
Physical Demands:
- Considerable reaching, stooping, bending, kneeling, and crouching
- Heavy physical effort (lift/carry up to 50lbs)
- Lifts, positions and/or transfers residents
- Occasional prolonged standing and walking
- Occasionally lift supplies/equipment
Working Conditions:
- Contact with residents/patients/visitors under wide variety of circumstances
- May be exposed to infections/contagious diseases
- Regularly exposed to risk of blood borne diseases
- Requires judgment/action which could result in the death of a resident
Pay Range: $23.00 - $29.03 hourly **Individual wages are determined based upon a number of factors including, but not limited to, an individual�s qualifications and experience
