At Hi-Desert Medical Center (HDMC) we've been caring for our community for over 40 years. Our Continuing Care Center (CCC), located on the campus of HDMC, provides specialized care to residents close to their own communities, with spectacular panoramic views of desert landscapes for both residents and their guests
The Continuing Care Center has 120 beds and offers three distinct levels of service in semi-private rooms:
Subacute care: This is the highest level of care available in our long-term care setting. The CCC provides 25 beds dedicated to residents with ventilator and tracheostomy requirements. Clinical staff is specially trained to meet the unique needs of these patients. The CCC is one of the few facilities in Southern California providing this level of care.
Skilled nursing care: We provide long-term skilled nursing care to residents who require assistance with personal and medical needs. Care is provided to our patients who require rehabilitative therapies, wound care or recovery from accidents or invasive procedures.
Long-Term Care: We provide long-term care to residents who require assistance with personal and medical needs.
Position Summary:
The Admission RN is responsible for maintaining census at or above budget. The Admission RN manages bed availability, accept and admits patients per established clinical capabilities. The Admission RN manages referral systems and responds to admission inquires in a timely manner. Provides administrative support to Administrator, interfaces with Clinical RN and handles referrals from all sources.

Primary Duties
· Performs admission of patients to the facility, including; Data Entry in EMR and development of baseline care plans.
· Screens admission, using approved criteria, for appropriateness of level of care and bed assignment and confirms room is ready, clean and properly set up for admissions.
· Coordinates insurance verification, checks previous skilled days, handles authorizations from insurance companies as needed.
· Performs an in-depth, systematic, pre-admission patient assessment and communicates pertinent information to appropriate healthcare providers.
· Ability to develop relationships with hospital discharge planners, families, and residents.
· Coordinates and/or assists in the patient admissions to CCC.
· Facilitates calendar and correspondence as needed to provide appropriate and timely communication among Hospital staff and the community.
· Handles a variety of special projects as assigned, which may be long or short-term projects, and contracts for the Post -Acute Team.
· Displays appropriate initiative/judgment.
· Consistently performs duties with minimal supervision, giving attention to detail, record-keeping and required paperwork inherent to admissions.
· Demonstrates competence in planning and time utilization of special projects.
· Consistently coordinates work to achieve maximum productivity and efficiency, demonstrating ability to recognize and establish priorities.
· Understands Hospital and departmental operations, policies and procedures, and applies to any situation.
· Understands basic functions of the department and readily identifies sources of information in the preparation of admissions.
· Clearly relates knowledge of Hospital and departmental operations, policies and procedures to others as requested and understands the organization and structure of the Hospital.
· Communicates problems effectively in an appropriate forum.
· Maintains functional filing system for methodical retrieval of documents.
· Supervision of admission assistant
· Other duties as assigned

Minimum Education
• Associates in Nursing
Certification/Registration
• Active BLS
• Active and unrestricted Associate Registered Nurse in State of California
Preferred Experience
• 1-year recent experience working as RN Admission Coordinator in a Long Term Care facility
• Experience in Microsoft office programs

Pay Range: $40.89 - $63.37 hourly **Individual wages are determined based upon a number of factors including, but not limited to, an individual’s qualifications and experience
