At Hi-Desert Medical Center (HDMC) we've been caring for our community for over 40 years. Our Continuing Care Center (CCC), located on the campus of HDMC, provides specialized care to residents close to their own communities, with spectacular panoramic views of desert landscapes for both residents and their guests
The Continuing Care Center has 120 beds and offers three distinct levels of service in semi-private rooms:
Subacute care: This is the highest level of care available in our long-term care setting. The CCC provides 25 beds dedicated to residents with ventilator and tracheostomy requirements. Clinical staff is specially trained to meet the unique needs of these patients. The CCC is one of the few facilities in Southern California providing this level of care.
Skilled nursing care: We provide long-term skilled nursing care to residents who require assistance with personal and medical needs. Care is provided to our patients who require rehabilitative therapies, wound care or recovery from accidents or invasive procedures.
Long-Term Care: We provide long-term care to residents who require assistance with personal and medical needs.
Position Summary:
Primary function of the position is to provide transportation services for residents of the Continuing Care Center. The position may assume a “split shift” format due to the nature of transport needs, yet may require more than eight (8 hours in a day if needed. Other transport needs of the Hospital may also be realized through this position, as time in the day allows, and as to not interfere with transport needs of the facility

Primary Duties
- Transports residents to and from the facility.
- Schedules all passenger pick up times and in doing so, calls passengers for the most efficient use of time and gas.
- Manages daily/weekly schedule so as not to exceed 40 hours without Director’s approval.
- Maintains professional boundaries, appropriate interaction and effective communication at all times.
- Immediately reports any resident problems to the Director or Charge Nurse/staff on duty.
- Ensures that physician writes a progress note of the visit and that this information is passed to the charge nurse.
- Complies with and enforces all van policies as established by the facility and Director.
- Complies with all laws governing the streets and highways of the State of California.
- Maintains van performance and upkeep by reporting to the Director and/or Engineering maintenance supervisor.
- Shall be responsible for daily/monthly maintenance check as assigned by the Director or Engineering maintenance supervisor.
- Shall be responsible for daily gas fueling of the van. All gas receipts must be “signed off” by the driver and turned in to the Director along with the credit card bill from Finance at the end of each month.
- Demonstrates competence in the care of young adult, middle adult and geriatric adult (when/if assigned to these patients/residents) relative to determining each patient’s/resident’s age-related and other physical, psychological and cultural needs and carrying out patient/resident care activities. Patients/Residents are treated with sensitivity according to their needs and age. Patient/Resident Rights are respected.
- Other duties as assigned, which may include, but is not limited to:
- Assist program staff with residents that may be difficult to handle, hostile, etc.
- Transports residents to the appointments.

Qualifications
Job Specifications
Physical demands:
- Light physical effort (lift/carry up to 10 lbs)
- Push and pull wheelchairs and/or gurneys with patients weighing up to 350lbs.
- Bend and crouch
- Ability to manually secure wheelchairs to floor of transport van
- Assist residents with 1:1 manual transfers to and from wheelchairs
- Sitting for extended periods
- Manual dexterity and mobility
- Occasional reaching, stooping, bending, kneeling and crouching
- Occasional standing/walking
- Occasionally lifts supplies/equipment
Working Conditions:
- Exposed to hazards from electrical/mechanical/power equipment
- May be exposed to toxic chemicals
Minimum Education/Certification/Experience:
- High School or equivalent
- Six months to one year experience driving a twelve (or larger) passenger van
- Maintains current, appropriate DMV licensure, Class C California Driver’s license or Class B (if necessary) certification required for employment. May be required to have a CDL with a passenger transport vehicle (PV) endorsement.
- Certified Nursing Assistant Certification
- BLS card required within probationary period.
- Clean DMV Record
- No prior traffic infractions
- No misdemeanors
- No felonies i.e., DUI etc

Pay Range: $23.00 hourly **Individual wages are determined based upon a number of factors including, but not limited to, an individual’s qualifications and experience
