Description:
At Hi-Desert Medical Center (HDMC) we've been caring for our community for over 40 years. Our Continuing Care Center (CCC), located on the campus of HDMC, provides specialized care to residents close to their own communities, with spectacular panoramic views of desert landscapes for both residents and their guests
The Continuing Care Center has 120 beds and offers three distinct levels of service in semi-private rooms:
Subacute care: This is the highest level of care available in our long-term care setting. The CCC provides 25 beds dedicated to residents with ventilator and tracheostomy requirements. Clinical staff is specially trained to meet the unique needs of these patients. The CCC is one of the few facilities in Southern California providing this level of care.
Skilled nursing care: We provide long-term skilled nursing care to residents who require assistance with personal and medical needs. Care is provided to our patients who require rehabilitative therapies, wound care or recovery from accidents or invasive procedures.
Long-Term Care: We provide long-term care to residents who require assistance with personal and medical needs.
Position Summary:
Provides nursing and wound care on the SNF unit.

Hours:
Primary Duties:
1. Start's IV's and obtains blood samples.
2. Updates care plans as needed.
3. Communicates the need for equipment or supplies to Charge RN or Unit Secretary
4. Completes assigned admission documentation and assessments.
5. Compiles and ensures others comply with all resident privacy and HIPPA requirements.
6. Ensures CNA's complete assigned resident care tasks.
7. Participates in monthly recapitulation process as requested.
8. Ensure DPAC/Advance Directive wishes carried out in accordance with CCC policy and procedure.
9. Provides resident and family education and training as assigned
documentation
10. Participates in resident Care Plan meetings and ensures care plans are appropriate and up-to-date.
11. Participates in Nutrition/Weight/Wound meetings
12. Assesses weekly measure of wounds
13. Contacts MD's for appropriate treatment orders
14. Assesses new admits
15. Ensures orders are appropriate for treatment C/P skin problems
16. Treatment nurse responsible fot the "m" section of the MDS
17. Orders specialty beds appropriate to criteria of wounds
18. Measure Decubs weekly, preferably on Sunday. Issues treatment if necessary, fills out decubitus reports, and distributes.
Position Specifications:
Minimum Education/Certification/Experience:
· Current California Registered Nurse License(RN) or Licensed Vocational Nurse (LVN)
· BLS Certification
· IV/Phlebotomy certification an advantage
Physical demands:
· Considerable reaching, stooping, bending, kneeling, and crouching
· Frequent prolonged standing/walking
· Heavy physical effort (lift/carry up to 50 lbs)
· Manual dexterity mobility
· Lifts, positions, and/or transfers residents
Working Conditions:
· Contact with residents/patients/visitors under a wide variety of circumstances
· May be exposed to infectious/contagious diseases
· Regularly exposed to risk of blood borne diseases
· Requires judgment/actions with could result in the death of a resident
