1. Accountable for conducting in-services on an ongoing basis to Case Managers and other hospital personnel regarding social service needs of patients.

2. Accountable for ensuring that social service needs are being met and documented.

3. Accountable for communicating with appropriate agencies regarding referrals and acts as a resource for Case Managers.

4. Accountable for preparation of written policies and procedures and the development of forms and formats that meet the needs of the hospital necessary for operations in the social service area.

5. Accountable for monitoring a variety of logs and records necessary for the proper documentation of department activity.

6. Accountable for participating in meetings and committee as assigned.

7. Accountable for maintaining good communications and interpersonal relations with patients, visitors, physicians and hospital employees.

8. Accountable for maintaining a standard level of attendance.

9. Accountable for demonstrating current knowledge of hospital and departmental policies

10. Accountable for following through on completion of assignments, overcoming obstacles and increasing efforts when necessary.

11. Accountable for meeting performance expectations for productivity, volume, timeliness and quality.

12. Accountable for originating, developing and implementing new methods to accomplish objectives. Generates innovative ideas to enhance overall operations.

13. Accountable for applying technical and/or professional knowledge to complete assignments.

14. Accountable for conveying information in a concise, accurate and understandable manner. Displays attentiveness, tact, and diplomacy in responding to others.

15. Accountable for conveying written information accurately and in a clear, concise manner.

16. Accountable for adjusting to changing environments, priorities and/or ideas. Demonstrates flexibility and willingness to accept additional responsibilities.

17. Accountable for planning and organizing by assessing responsibilities and established priorities in order to allocate time and materials.

Job Summary

Responsible for the overall provision of Social Services. Ensures that social services include identification of social deterrents that may interfere with the patient's compliance with the medical treatment plan. Provides direct intervention to patients, families or employer by the referral process. Conducts in-services for Discharge Planners, Case Managers and other hospital personnel regarding social service needs of patients. Performs other duties as needed.

Education:

  • Required: M.A. / M.S.
  • Major Social Work

Licenses / Certifications

  • Current California license as a Licensed Clinical Social Worker (LCSW).

Experience

  • 2 -3 years

Skills / Knowledge / Abilities

  • Analytical and grammatical skills are necessary to communicate effectively, verbally and in writing. Must be fluent in spoken and written English. Must be able to fulfill the essential functions of the position.
