Job Summary
The individual in this position will be an advanced clinical practitioner having additional expertise in a specific field of pharmacy practice. This individual will lead clinical specialized services offered by the department, having additional training, certification, and/or experience (e.g. antimicrobial stewardship, diabetes management, heart failure clinics, pain management, hematology/oncology, etc.). This pharmacist will function in a staff pharmacist role as needed.

Position Accountabilities
1. Performs assessment of patient therapy that demonstrates knowledge and consideration of age specific and cultural differences. Makes recommendations to modify therapy to meet the age specific requirements of the newborn (0 to 30 days), pediatric (1 to 12 years), adolescent (13 to 17 years), adult (18 to 70 years), and geriatric (71+ years) patients.
2. Provide direct patient-centered care and integrated pharmacy services in collaboration with physicians, nurses, and other healthcare professionals.
3. Deliver medication therapy management within specialized patient care areas and ensure pharmaceutical care programs are appropriately integrated throughout the institution.
4. Assist in development and implementation of facility-specific protocols, guidelines, and reference documents related to medication therapy management.
5. Conduct Medication Use Evaluations (MUEs) and assist in making necessary changes to pharmacotherapy practice as needed based on MUE findings.
6. Serve as a pharmacotherapy educational resource for physicians, nurses, pharmacists, patients, families, hospital departments, and external groups as needed.
7. Assist with development and maintenance of pharmacy staff competencies.
8. Serve as a preceptor for pharmacy students and pharmacy residents
9. Performs Pharmacist duties including but not limited to:
• Screens and interprets chart orders or prescriptions for drug interactions, therapeutic duplications, allergic or adverse drug reactions, and proper dosing relative to patient age, weight and disease state.
• Dispenses medications written on chart orders or prescriptions.
• Maintains current, complete, and accurate patient medication records.
• Oversees preparation of parenteral nutrition solutions, intravenous admixtures, chemotherapy, and piggyback solutions.
• Verifies unit dose carts, IV orders, piggybacks, parenteral nutrition and chemotherapy orders prepared by the pharmacy technician, intern, or another pharmacist as required.
• Verifies entry of chart orders into the computer system by the pharmacy technician or intern.
• Rectifies any missing doses or missed doses.
• Provides drug information to physicians, nurses, pharmacists, and other health professionals.
• Reviews medication profiles and monitors patient drug therapy upon entering new orders.
• Performs all other duties as assigned.
Qualifications
Position Qualifications
Minimum Education
- Doctorate of Pharmacy or Bachelors Degree in Pharmacy with 5 yrs experience. Advanced degree (MS, MBA, MHA) and or pharmacy residency is preferred.
Minimum Experience/Skills
- Residency training or 3 years experience in hospital setting.
- Minimum of 2 years in supervisory position.
- Experience with medication management, policy/procedure/ protocol development, staff supervision (clinical and non-clinical), process improvement strategies, and medication safety practices.
- Knowledge of regulatory and accreditation bodies, survey standards, and outcomes metric
License/Certificates/Credentials
Must possess a valid pharmacist license as provided by the State of California and must be in good standing. Preferred qualifications include:
- Advanced certification or equivalent training (e.g. BCPS, CGP, ACLS, etc).
- Competent use of Office with experience in performance improvement planning
