Position Accountabilities
1. Follows the multidisciplinary, goal oriented care plan for each patient in collaboration with members of the health care team and provides care based on that plan.
2. Ensures patient safety b observing and proactively implementing safety measures with specific consideration of patient’s functional and developmental age
4. Competent in all routine nursing skills within the LVN scope of practice, including: (1) assistance with activities of daily living, (2) medication administration, (3) management of indwelling catheters and drains, and (4) safe use of any medical equipment (as appropriate).
5. Demonstrates the ability to provide care in emergency situations and follows established emergency procedures.
6. Performs physical and psychosocial observation of all age groups of patients that demonstrates knowledge and consideration of age specific and cultural differences. Modifies nursing care to meet the age specific requirements of the newborn (0 to 30 days), infant (31 days to 1 year), pediatric (1 to 12 years), adolescent (13 to 17 years), adult (18 to 70 years), and geriatric (71+ years) patients. Knowledge and understanding of differences in care including, but not limited to: growth and development, medications, and special physical and psychosocial needs. Modifies nursing care as appropriate.
7. Additional responsibilities are outlined on the Clinical Ladder and are to be documented on the Annual Evaluation Checklist.
8. Demonstrates strict adherence to COBRA/EMTALA requirements.
9. Demonstrates, through practice, understanding of job description and functions only within scope of practice. Limits verbal orders to emergency situations and assures inclusion of all verbal orders on the final medical record.

Job Summary
Reporting to the Emergency Director of Patient Care Services/Shift Manager of the assigned area. Performs duties under the direction of the Registered Nurse (RN), providing high quality, individualized patient care using the components of the nursing process.
Position Qualifications
Minimum Education: Associate Degree with focus in Vocational Nursing
Minimum Experience/Skills: 1+ year experience in direct patient care in a Long Term Acute Care or Skilled Nursing Facility.
License/Certificates/Credentials: Current California LVN license; Current BLS certification IV certification required or must obtain within 90 days of date of hire or transfer.
