POSITION SUMMARY

Under a Registered Nurse’s supervision, provides nursing care for a group of patients, based on matching the patients’ needs with the LVN’s competencies. Completes established competencies for the position within designated introductory period. Other related duties as assigned.

Responsibilities:

• Provides nursing care using the nursing process under the direction of the Registered Nurse; fulfills 
role within defined role expectation of Board of Nursing and BHS
• Reports pertinent patient information to Nurse(s) and documents according to established guidelines
• Implements plan of care by providing and evaluating individualized nursing care
• Provides for the physical comfort and safety of patients, visitors, and members of the health care 
team including infection control and handling of hazardous materials
• Documents all plans and care provided
• Prioritizes and sequences work activities and delivery of nursing care to patients that ensures the 
patient’s comfort, safety and progress toward established goals
• Calculates accurately and administers safely medication within scope of practice
• Coordinates and collaborates with multidisciplinary team members in the planning and management 
of patient care
• Participates in discharge planning; provides instruction to patient/family
• Maintains and promotes professional standards and development by participating in inservices, 
committees, and staff meetings.
• Demonstrates commitment to Performance Improvement
• Serves as a role model for compassionate care and service excellence 
• Recognizes and reports changes in patient condition
• Manages patient hand-offs at shift change and between department and facilities; utilizes 
documentation tools for hand-offs as required
• Facilitates patient movement within the department and through the facility, proactively supporting 
timely admissions, discharges and transfers
• Provides every patient the defined standard of care 
• Utilizes resources efficiently and effectively
• Maintains safe environment
• Addresses patient population-specific needs
• Participates in Performance Improvement activities 

Qualifications

MINIMUM EDUCATION: Graduate of an accredited school of vocational nursing.

MINIMUM EXPERIENCE: None

PREFERRED EXPERIENCE: LVN in an acute care setting.

REQUIRED CERTIFICATIONS/LICENSURE: Possession of current Texas State License for Licensed Vocational Nurse

REQUIRED COURSES/COMPLETIONS (e.g., CPR):

  • Must have active healthcare provider Basic Life Support on hire (ARC or AHA); however, must have AHA Healthcare Provider BLS within 60 days of hire.
  • AHA PALS must be completed within 60 days of hire or transfer into role (EMERGENCY ROOM)
  • S.T.A.B.L.E. – Level 2 Nursery-Must provide proof of prior completion or obtain within 6 months of hire or transfer date into unit (Renewal is not required).
  • Non-Violent Crisis Intervention must be completed within 60 days of hire or transfer into role (BEHAVIORAL HEALTH and EMERGENCY ROOM)
