Job Summary

The quality coordinator is responsible in helping to drive all quality initiatives to ensure successful compliance with all payers, internal and governmental incentive programs. Helping the market to achieve maximum incentive payments. This position also supports all safety, patient experience, and other ad hoc initiatives, as necessary.

Responsibilities
  • Reviews non-compliance lists and conducts outreach to ensure timely disease specific and wellness visits and tests with submission to the respective payer.
  • Completes quality, safety and other ad hoc reporting on a monthly basis.
  • Prepares payer continuity of care forms for provider completion.
  • Reviews monthly payer reports related to medication adherence, open care gaps, and HCC/Raf opportunities and communicates outliers to respective practice to complete.
  • Participates in practice safety walkthroughs and proactively helps to identify potential areas of risk.
  • Help to facilitate new hire and annual competency assessments.
Qualifications

Education:

Required: Must have a high school diploma/GED.
Preferred: Associates degree.

Experience:

Required: 2 years working in an outpatient medical practice.
Preferred: Experience in primary care setting.
