Job Summary

Position Summary

Working under the supervision of a Registered Nurse, the Constant Observer supports patient safety through monitoring and observation of assigned patient or patients. Provides continual bedside observation to assigned patient. Ensures patient safety. Intervenes as necessary to prevent physical harm to patients who might be at high risk to injure themselves or others due to confusion, delirium, self-destructive behavior, or a has a high predictive factors of falling. Works with others in the healthcare team to assure prompt intervention to maintain patient safety.

Responsibilities

  • Remain at patient bedside at all times.
  • Remove visible hazards and/or potentially unsafe equipment from the immediate environment.
  • Report unsafe/broken equipment or conditions to immediate supervisor.
  • Accompany patient to procedures/tests/therapy as needed.
  • Promptly refer to supervising RN any signs or symptoms that may indicate a change in patient’s condition.
  • Assist in calming or reorienting patient in cases of agitation or confusion.
  • Communicate in a positive professional manner; develops and asks questions which open up the dialogue. Provide clear, direct and specific instructions or information to others. Listen with patience and empathy.
  • Observe and respect of patient’s right to privacy.
  • Maintain quiet environment.
  • Adhere to professional conduct/dress.
  • Maintain orderliness of patient rooms.

Qualifications

  • Minimum Requirements:
    • Experience: 
      • Must possess effective interpersonal skills. 
      • Must be able to follow written and verbal instructions. 
      • Must be able to work variable shifts, holidays and/or weekends. 
      • Must be able to transport self to alternative work sites. 
      • Within 90 days of hire completes all .Edu courses on duties of the Constant Observer and the .Edu courses on restraint and seclusion. 
      • Completes hospital approved training in managing patient behaviors and de-escalation techniques. 
      • Constant Observers providing patient care must have completed all applicable
      • Competencies within 90 days.
    • Licensure/Certifications: CPR Certification.
  • Preferred Requirements: 
    • Education: High School Diploma/Equivalent
    • Experience: 
      • Knowledge of hospital environment desired. 
      • Previous hospital and/or health care related experience preferred. 
      • Bilingual, English/Spanish helpful.
