Responsibilities
Job Duties
1.See all new patient referrals and document assessment and recommendations in the chart in Progress Note section.
2.Make recommendations for topical therapy based on hospital protocols, research-supported best practice and current standards of care. Obtain MD order to initiate treatment at that time if possible.
3.Monitor patient outcomes and progress on an ongoing basis or as requested by MD or indicated by change in wound/skin/ostomy status
4.Make recommendations for cost effective wound/ostomy management, based on hospital protocols, research-supported best practice and current standards of care, for patients after discharge from acute care setting. Participates in discharge planning activities.
5.Serve as a clinical resource to patient care providers for wound/skin/ostomy care issues
6.Participates in multidisciplinary, patient-centered case conference as appropriate.
7.Participates in research projects related to WOC issues.
8.Complies with all Federal and State regulations and Joint Commission requirements in the provision of care.
9.Attends all department-specific staff meetings, patient care conferences, and inservices as requested.
10.Completes other duties as assigned.
Qualifications
- Current California RN License.
- BSN preferred, Master’s preferred.
- At least 2 years of experience with wound ostomy patient care.
- Completion of an accredited Wound Ostomy Continence ET Nurse Program (WOCN) certification or Wound Ostomy Specialist
- (WOS) certification or Certified Wound Care Nurse (CWCN) certification.
- Excellent communication skills and the ability to work well with others.
- Good organizational skills.
- Good judgment and problem solving skills.

Position Summary
Serves as a clinical resource to EMC staff on issues pertaining to patients with wounds, pressure ulcers, ostomies, fistulas and incontinence. Provides direct patient care for evaluation and education purposes. Establishes and maintains practice standards and provides ongoing staff education.
