Description:
Position Summary
Facilitates and assists patients through the healthcare continuum by being a resource for all aspects of the patient experience for orthopedic, neurosurgical and bariatric patients. This role includes providing education and support to patients, family, staff and caregivers. The goals of this position include improving patient care and flow, eliminating barriers and improving the overall patient experience by connecting the patient and family with appropriate resources and services. Establishes navigator relationship with patients from referral, through surgery, to post-op, to discharge.
Coordinates referrals from physicians and health care facilities for orthopedic, neurosurgical and bariatric patients;provides a significant level of patient education related to their elective procedure and planned treatment; develops patient care pathways and educational seminars; ensures all pre-surgical requirements are completed as scheduled.

- RN. Florida Licensure. Minimum of three years nursing experience; post-operative care experience preferred or case management experience.
Licensure / Certifications
- Other: Critical thinking and problem-solving skills; Assessment skills to include the assessment of psychosocial needs; the ability to plan and organize time; the ability to effectively utilize resources to meet patient needs. Important skills to have: Relationship management and strong professional communication skills.
Minimum Experience
- Experience – minimum 3 years as Post –op or Perioperative nurse
