Job Summary: Structural Heart Coordinator

As a Structural Heart Coordinator in the clinical setting of Cardiology and Cardiac Surgery, you will play a crucial role in coordinating and managing patient care related to structural heart procedures. You will work closely with a multidisciplinary team of clinicians, including cardiologists, cardiac surgeons, anesthesiologists, nurses, technicians and device company representatives, to ensure seamless and efficient care delivery. Your primary responsibilities will include:

1. Patient Management:

- Facilitate patient referrals and coordinate appointments for diagnostic testing, consultations, and procedures.

- Serve as the main point of contact for patients and their families, providing education and support throughout the treatment process.

- Collaborate with clinicians to develop individualized care plans based on patient needs and treatment goals.

2. Procedure Coordination:

- Coordinate scheduling of structural heart procedures, ensuring all necessary pre-procedural evaluations and preparations are completed.

- Arrange the availability of resources, such as equipment, medications, and surgical suites, to support safe and effective procedures.

- Communicate with patients, physicians, and other healthcare professionals to ensure clear understanding of procedural details.

3. Data Management:

- Collect and maintain accurate patient data related to structural heart procedures for documentation, research purposes, and quality improvement initiatives.

- Ensure compliance with regulatory requirements regarding data privacy and confidentiality.

- Generate reports summarizing procedural outcomes and participate in data analysis for performance improvement projects.

4. Team Collaboration:

- Collaborate effectively with members of the multidisciplinary team to optimize patient care outcomes through timely communication and coordination.

- Prepare Powerpoint presentations for and participate in team meetings by discussing patient cases, sharing knowledge, and contributing to decision-making processes.

- Act as a resource person for colleagues by sharing expertise on structural heart procedures and best practices.

Qualifications:

- PA or NP degree.

- Minimum of 2 years experience in the field of Cardiology or Cardiac Surgery is preferred.

- Strong organizational and coordination skills to manage multiple patients and procedures concurrently.

- Excellent interpersonal and communication skills to interact effectively with patients, families, and healthcare professionals.

- Proficiency in medical record documentation and familiarity with electronic health record systems.

- Ability to work collaboratively in a team-based environment and maintain a proactive attitude towards professional development and continuous learning.
