Onboarding Process: Please be advised that candidates must successfully complete a background check and pre-employment health screening which includes a drug screen.
Position Summary:
Utilizes an interdisciplinary approach to organize, coordinate, monitor, evaluate and modify plans of care, focusing on complex medical and psychosocial needs of the Hospital’s patients and their families. Performs pre-admission screening review of patients. Assesses and plans interdisciplinary plan of care that effectively manages patient along the continuum. Ensures appropriate use of resources and coordinates discharge planning.
Qualifications:
EDUCATIONAL REQUIREMENTS:
RN, licensed in Massachusetts
BSN or commensurate experience
Case Management Certification preferred
Computer literacy in Word, Excel, Outlook
EXPERIENCE REQUIRED:
5 years clinical experience Required
Minimum 1 year Case Management Experience Required
Case Management Certification preferred
Working Knowledge of InterQual Criteria preferred
Outcomes/resources management skills
Midas and MediTech Experience preferred
PROFESSIONAL LICENSURE AND CERTIFICATION REQUIRED:
RN, licensed in Massachusetts
BSN or commensurate experience
Case Management Certification preferred
Computer literacy in Word, Excel, Outlook
INTERPERSONAL RELATIONSHIPS:
Communicates effectively with patients, families, visitors, and all members of the health care team.
PHYSICAL REQUIREMENTS:
Physical health sufficient to meet the ergonomic standards and demands of the position and its location.

REPORTING RELATIONSHIPS AND LEVEL OF AUTONOMY:
Director of Case Management.
