A specialty referral hospital, Harper has established Centers of Excellence in neurosciences, bariatric surgery, vascular services, multidisciplinary cardiovascular services, hypertension, stroke prevention and heart failure.
Summary
Under general supervision, and according to established policies and procedures, coordinates the clinical, research, educational and administrative functions of the designated Epilepsy, Neuromuscular, Stroke and/or Multiple Sclerosis Program(s) for Harper University Hospital. Coordinates and monitors the operational activities of the assigned program(s) including monitoring expenditures, developing and monitoring budget, preparing reports related to the program(s), and developing quality assurance measures.
Acts as a member of the care management team by assessing, developing, implementing and monitoring a complete plan of care with patient and family in conjunction with physician and outside agencies.
On a concurrent basis, assesses level of care, diagnostic testing and procedures performed, quality and clinical risk issues, and documentation for medical record completeness.
Communicates with patient, family, medical team, research, caregivers and third party payers as necessary. Performs insurance verifications/authorizations.
Schedules home care and infusions, and care at other institutions as needed. Monitors medications compliance and complications for therapeutic results. Involved in financial assistance and utilization of available resources. Functions as an admissions coordinator for new admits and transferred patients. Formulates, implements and evaluates educational strategies for patient and family.
Develops and directs educational seminars. Analyzes case management outcome data to identify issues and make recommendations. Maintains current knowledge of health care techniques and practices by participating in educational programs. Maintains a working knowledge of the requirements of the payers most frequently seen with the patient population.
Provides input into procedure/protocol development, ensures compliance and provides interpretation of intent for clinical support activities and staff. As directed, provides input into policy development from a professional, technical or procedural perspective.
Assists in the implementation of new and existing programs. Develops daily, monthly, and/or yearly goals and measures for the program and, as requested, assists in assessment of goal attainment. As requested, provides input on performance appraisals and personnel actions such as hires, fires, disciplines, etc. Provides input into departmental budget.
Qualifications:
1. Bachelors degree in Nursing, or related clinical discipline, or the equivalent combination of education and experience. Masters degree in Nursing preferred.
2. Licensed to practice as a Registered Nurse in the State of Michigan.
3. Three to five years of progressively more responsible related health care experience, including clinical nursing experience. Previous experience with the targeted patient population preferred.
