Summary:
The Registered Nurse (RN) Transfer Center will perform delegated duties in the Transfer Center in order to meet the needs of the transfer process. Responsible for managing and coordinating all aspects of the transfer process. Responsible for coordinating direct admissions and hospital transfers to and from community hospitals with the intent to transfer each patient to an appropriate Tenet hospital. Works collaboratively to foster relationships with referring facilities, physicians, and hospital staffs in representation of Tenet Healthcare Mission and Values.
Responsibilities:
Collaborates with clinical staff, medical staff, and leadership teams to identify, trend and report barriers to patient access and throughput. Works directly with staff at each client facility to ensure timely access, appropriate bed assignment, and accurate patient information documentation to assist with an appropriate destination plan of care at receiving facility.
Qualifications:
Minimum Qualifications
1. Graduation from a state approved school of nursing or an accredited school required.
2. Academic degree in nursing (Bachelor's or master's degree) preferred.
3. 4 years of experience in an acute care setting required.
4. Experience in an ER/critical care or charge nurse, house supervisor, or other management experience preferred.
5. CPR Certified.
6. Must be currently licensed, certified or registered to practice profession as required by law, regulation in state of practice or policy.
