Description

Provides direct patient care to include assessment, planning, implementation and evaluation of assigned patients. Collaborates with other professional disciplines to ensure effective patient care delivery and achievement of desired patient outcomes.

Responsibilities:

  • Performs and documents comprehensive systematic patient assessments.  Identifies deviations from the norm and intervenes accordingly.  
  • Exercises scientific based clinical judgment and decision-making skills related to patient assessment, planning, implementation and evaluation.
  • Performs appropriate screens to assess patient nutritional, functional, fall-risk, suicide, abuse and discharge status.
  • Initiates, develops and maintains a written plan of care
  • Coordinates and supports the plan of care through communication with other members of the health care team.  
  • Supervises, delegates tasks/activities of other team members.
  • Integrates knowledge of policies and procedures into practice.
  • Initiates interventions or actions based on patient condition and plan of care.
  • Obtains, verifies, administers and documents medications.
  • Performs or assists in therapies and procedures.
  • Provides and documents interventions in a timely manner.
  • Identifies needs, customizes education for patients and families, and documents education as per hospital policy.  
  • Completes a minimum of 10 contacts hours per year.
  • Patient care activities are coordinated, communicated and delegated to team members appropriately.
  • Patient reports are communicated in a concise, accurate and timely manner in accordance with unit standards.
  • Evaluates care provided by direct reports.
  • Helps create a positive and adaptive work environment.
  • Maintains effective communication with co-workers and other departments to ensure the clarification and meeting of customer needs.
  • Promotes effective working relationships based on open communication, trust and respect.  
  • Consistently offers assistance to other team members.  
  • Establishes therapeutic relationships with the patient and/or significant other.
  • Participates in the evaluation of other team members.
  • Assists customers to locations throughout the hospital by providing clear and accurate directions or by personally escorting them when needed.  Utilize resources to obtain answers to customers’ questions.  

 

Qualifications

Minimum Education: Associate’s degree in Nursing required.

Minimum Experience/Additional Certification: RN-C or CCRN preferred

Required Licensure and Certifications: A valid Permanent Multi-State RN License from the state in which you reside is required. Should you hold a RN license from another Compact state, you are required to apply for and obtain a Multi-State RN License from the state in which you reside within 30 days. American Heart Association BLS required, must be obtained within 7 days of hire. ACLS and NIHSS required within orientation period.
