Social Worker II Case Management PRN Days Position Summary:
Provides an integrated continuum of care for patients from preadmission to post-discharge. Screens for discharge planning needs and initiates a plan for community resources and/or placement in appropriate post-discharge facility. Completes Individualized Treatment Plans. Provides group, individual and family therapies. Screens for discharge planning needs and initiates a plan for community resources and/or placement in appropriate post-discharge facility. Communicating with patients and their families around plan of care. Communicating and collaborating with physician and other team members.
THE SOCIAL WORKER II CASE MANAGEMENT PRN DAYS CANDIDATE WILL POSSESS THE FOLLOWING EDUCATION, LICENSE/CERTIFICATIONS, AND EXPERIENCE.

Minimum Education: Master’s degree in Social Work required.
Minimum Experience: 1 to 2 years of job-related experience preferred, including experience as a licensed clinical social worker, experience in an acute care setting, knowledge of federal and state laws and Medicare/ Medicaid calculations, and experience with computer software.
Required Certifications/Registrations/Licenses: SC licensure in Social Work required. American Heart Association BLS CPR certification required. CPI certification with orientation period.
