Position Summary

The Rehab Routine Care Service line Clinical Nurse Coordinator is a Registered Nurse, using critical thinking skills, coordinates and ensures safe, quality care to patients in a family centered care environment utilizing the nursing process. In addition, this RN maintains Best Practice by monitoring compliance through the Performance Improvement process. S/he serves as a resource to guide, educate, transition and mentor new hires into the rehabilitation specialty. S/he assists with bedside procedures, equipment set-up, and ensuring rehabilitation staff are adequately prepared and can apply knowledge and skills at the bedside. S/he assists with nurse, physician, and patient experience and assists them by navigating them through their rehabilitation stay to ensure the best possible experience. S/he educates staff to ensures competency and application at the bedside, and assists in the evaluation of the rehabilitation staff. S/he will also provide patient and family/caregiver education to ensure a safe discharge to home. S/he will work in conjuction with the rehabilitation leadership team to oversee the rehabilitation program for all persons served.
